Read the last column as a to-do list. In all three papers every question climbs the same ladder: (a) and (b) are Achieved, (c) and (d) are Merit, and (e) is Merit to Excellence. Excellence is only ever on the (e)s, and each of those is a full problem (a proof, an optimisation, a related-rates chase or a parametric inflection) built from the same rules as the (a)s.
Practice for this standard covers the whole topic — flashcards and quiz.
